A 24-year-old woman presented at 33 weeks gestation with severe left-sided abdominal pain and orthopnoea. Ventilation-perfusion scan demonstrated two segmental mismatched defects consistent with bilateral pulmonary embolism. Anticoagulation with enoxaparin was commenced.
